Help me Pls

I'm already angry. When the link with which any server in the lower left corner I see 'disconnected'. And so every other time. I have to go out and come back to this server. As the worst in the game will show up this information 'disconnected' .I have internet radio, with this I have two IP addresses which periodically change. 6655 Server Port. Client Port 6654, I do not know whether that is a meaning. Please help, thank you in advance. Regards
